This form is to be completed by candidates applying for water baptism and to become members of Lighthouse Evangelism.  Only 16 years old and above can be eligible to be a member.
此表格适用于即将接受洗礼并申请成为灯塔教会的会员。仅限于16岁及以上才有资格成为会员。

Please note that there is a limited capacity for each baptism session. Selection of a preferred time does not guarantee placement in that session. By submitting this form, I permit Lighthouse Evangelism to collect, use, disclose, or otherwise process my data for church administrative and other purposes by the Personal Data Protection Act (PDPA). 呈交此表格即表示我准许灯塔教会遵照个人资料保护法令（PDPA）在行政上收集；使用我的个人资料。
