In return for allowing me to voluntarily participate in this camp programme and all related activities, I understand and confirm my agreement:

1.	我愿意承担和接受此营会和所有相关活动可能所造成的任何伤害，损失或损坏之风险。
To assume and accept all risks and to be solely responsible for any injury, loss or damage, arising out of my participation in this camp programme and all related activities.
	
2.	我将在整个营会和所有相关活动中极力保护灯塔教会，其同工及代表免于承担任何第三方的个人财产损失或人身伤害以及可能产生的所有索赔、要求、行动和费用的责任。
To hold harmless, indemnify and release Lighthouse Evangelism, their employees and their representatives from any and all liability for any damage to the personal property of, or personal injury to, any third party and all claims, demands, actions and costs which might arise out of my participation in this camp programme and all related activities.	

3.	我将致力并完全遵守委员会所提供的所有安全指示。如果我无法与委员会给予配合，委员会保有让我自费且把我送返回国的权力。
To diligently observe and fully comply with all safety instructions given by the committee. In the event that I do not co-operate with the committee, the committee reserves the right to send me home at my expense.

我同意灯塔教会及其筹委会将不会对活动过程中可能发生的任何意外事件承担责任或索赔。I agree that my child's participation in the activity is purely voluntary. I will not hold the Church liable or responsible for any loss or injury sustained by my child arising in connection with his/her participation in the programme.
